What to expect at your initial appointment?
Your initial appointment will be up to 75 minutes in duration. We start by discussing your health and other factors that may be contributing to your concerns. After completing a thorough health history, physical exams may be will be performed as necessary. Depending on your concerns, we may also discuss lab testing; however, this is not always necessary. The last portion of your appointment will be spent discussing treatment options available to address your concerns.
how to Prepare for your first appointment?

You will receive an email once your appointment is scheduled prompting you to complete online intake, consent, and direct billing forms (if applicable). Complete the online intake and consent forms at least 24 hours prior to your initial appointment. We understand that the forms are detailed and take time to complete, but they are incredibly useful in helping to determine the root cause of your concerns. If completed in advance, the entire intake form is reviewed prior to your appointment.

Please bring all natural health products you are taking to the appointment with you or take pictures of the label.

Bring a paper copy of all lab test results and imaging reports performed within the last 2 years, and prior if there have been additional completed that are relevant to your current concerns. This information is helpful, but not absolutely required. Your appointment will proceed as planned if you cannot bring this information with you.
